I think people who care about clothes tend to take take pride in their appearance and because the way we dress sends a nonverbal message of how we want the world to perceive us, dressing well is an important vehicle for expressing self-pride.  As to whether self-pride equals narcissism, I suppose it depends on whether it is taken to extremes to the exclusion of caring about the welfare of others.
