He's got kidney problems, check the pigmentation around his eyes. The Duke of Westminster has the same.
And his suit looks less than flame retardant to me.

What strikes me first is the lack of texture in all pieces of the outfit. Combine that with the ostensibly poor fit, ugly shirt collar, and low notches, and I think "cheap".
