Interlinings used by bespoke shirtmakers are usually quite porous (more than the shirt cloth itself in many instances) and the polymer used to stick them to the fabric ends up being so thin, it shouldn't create any discernable ventilation problems.
I enjoy wearing both fused and unfused collars and cuffs. It all depends on the type of shirt and occasion. My casual shirt collars (BDs, some of the open-collar "pajama" versions) are usually fused, more formal collars unfused --this may seem counter-intuitive at first but it works for me. Barrel cuffs always fused; it looks neater. French cuffs always unfused --fusing would make them look "dead"-- and always accompanied by an unfused collar.
l always insist on combination heals, but the results are always hashed mash. Pics later.
