Bosthist, this is fantastic.

See, here we can drop all the various sub-genres of styles and just say that what is suggested here is good sense.

It is too bad that, despite evolutions in fabric, cut, and color, etc. this basic kit could not have remained the same in overall description despite those changes.

So SENSIBLE. So straightforward. So unlike the guides one sees today.

It's hard to find fault with any of it.

I'm also strongly in agreement with that bit on tipping. So many seem to overtip. I have been criticized by fellow diners for leaving 15% before. I leave 10% at expensive restaurants...I reason (perhaps wrongly) that the waitstaff is not only paid more but that even 10% when the tab is over $100 is a pretty nice draw.
