I have another problem. 

My neck is 16.5. 

It's easy to find 33 sleeves, but the problem is that generally in a 16.5/33, the shirt isn't useful as something to wear with a suit.

It's useful as a tent, maybe a parachute.

That's why I go bespoke.  Hemrajani generally does the trick, but I'd love to give Raphael a whirl, he seems to be a magician.
