The Johnston and Murphy headquarters are located within an easy drive of where I live.  I am very interested in learning more about their Custom Select program.  The shoes are made in the United States; maybe even in Nashville, TN.  I don't know whether or not they are machine made or handmade though, as I suspect that much like the situation with Bass, the artisans who formerly made the shoes were laid off long ago.  If I am able to learn more, or even take a tour of the factory, I will try to post pictures.
